Ardennes Campaign Medal: Complete "Ardennes Campaign" under the Normal difficulty setting. 
Normandy Campaign Medal: Complete "Normandy Campaign" under the Normal difficulty setting. 
Tunis Campaign Medal: Complete "Tunis Campaign" under the Normal difficulty setting. 
War Hero: Complete the game under the Elite difficulty setting. 
War Veteran: Complete the game under the Veteran difficulty setting. 

Bonus weapons:
Successfully complete the game under the Normal difficulty setting to unlock the M1918A2 Light MG, M7 Grenade Launcher, and Tank Destroyer vehicle (only during tank missions).
